This particular blade is renowned for offering easier access to the laryngeal inlet, significantly improving the visual field for the practitioner.

These blades are especially useful in adult intubation, where they are inserted into the right side of the mouth to adeptly displace the tongue laterally. The tip of the blade rests in the vallecula, a space in the throat, and is then lifted forward. This action elevates the epiglottis, effectively exposing the laryngeal inlet for intubation. Importantly, their design aims to minimise the risk of dental injury, making the procedure safer for patients.

How to use Macintosh Laryngoscope Blades

  1. Positioning the Patient: Ideally, place the patient’s head level with the lower tip of your breastbone (xiphoid process) for mechanical advantage. Use your shoulders and upper back to lift, ensuring good ergonomics and personal injury prevention.
  2. Intubation Begins with Head Positioning: Start by tilting the head into extension with your right hand to align all axes, then anchor it using your left hand.
  3. Open the Mouth Fully: With the head anchored, use your right hand to open the patient's mouth. Place your thumb on the lower jaw and your middle finger on the upper jaw, pushing rather than spreading to open wider. Position your fingers far to the right for ample space.
  4. Prepare for Blade Insertion: Step back, and with your left hand, pick up the blade. Hold it lower down on the handle, with the heel of your hand at the junction between blade and handle, to fine-tune the blade angle.
  5. Insert the Blade Delicately: Insert the blade gently to the right of the tongue, avoiding contact with the teeth. Use a tilting motion if necessary, then sweep the blade around the right side of the tongue.
  6. Avoid Lip Injury: Use your right index finger or thumb to keep the lips away from the blade and teeth.
  7. Identify the Epiglottis: Slowly advance the blade with your left hand, sweeping the tongue to the left. Begin to lift slightly and look for the epiglottis. Only fully control the tongue after identifying the epiglottis tip.
  8. Final Placement and View: Once the epiglottis is visible, sweep the tongue left and lift upwards and away for a clear view of the laryngeal inlet.
